Recovery Centre for Addictions, Trauma and Families respects the privacy of clients and donors by protecting personal information and adhering to legislative requirements with respect to protecting privacy. Recovery Centre for Addictions, Trauma and Families values the trust of all individuals with which it relates and recognizes that maintaining this trust requires that Recovery Centre for Addictions, Trauma and Families be accountable in how it collects, uses and discloses personal information. Disclosure is also governed by the Code of Ethics of the Professional Association of Canadian Christian Counsellors (PACCC).* Personal information is information about an identifiable individual. It does not include business contact information and publicly available information.
Recovery Centre for Addictions, Trauma and Families follows the following 9 principles of fair information practices set out in the Personal Information Protection and Electronic Documents Act. 1. Accountability Staff, volunteers and independent contractors are responsible for the collection, processing and safeguarding of personal information under their control and will follow established privacy protection practices. 2. Identifying Purpose Recovery Centre for Addictions, Trauma and Families needs to collect, use and disclose information about its clients in order to provide service and deliver its programs. The purposes for collecting information are:
- To manage, develop and enhance Recovery Centre for Addictions, Trauma and Families operations, programs and services.
- To assess, process and collect applicable fees for service.
- To assess client needs and determine appropriate action plans.
- To determine program or service eligibility.
- To provide safe and secure work environments.
- To collect data for statistical and research purposes.
- To understand the changing needs of the communities we serve.
- To meet legal, regulatory and contractual requirements.
Employees and volunteers of Recovery Centre for Addictions, Trauma and Families shall make reasonable efforts to explain the identified purposes for collecting information and shall not use or disclose personal information for any new purpose without the consent of the individual. Recovery Centre for Addictions, Trauma and Families needs to collect information about its clients and donors in order to receive payment for services, acknowledge gifts, issue receipts, develop donor relationship and meet legal requirements. Recovery Centre for Addictions, Trauma and Families does not sell, share, trade or rent out donor information.
3. Consent The knowledge and consent of an individual is required for the collection, use or disclosure of personal information, except where the use or disclosure is required by law. The following actions by an individual constitute implied consent for Recovery Centre for Addictions, Trauma and Families to collect, use and disclose personal information:
- Application for services in writing, by telephone or over the internet.
- Completion of a donor pledge form or submission of a donation.
Express consent is required for an individual to disclose any information to anyone who is not a staff member, volunteer or independent contractor of Recovery Centre for Addictions, Trauma and Families.
4. Limiting Collection The collection of personal information shall be limited to that which is necessary for the purposes identified by Recovery Centre for Addictions, Trauma and Families. Information will usually be collected directly from the individual about whom the personal information pertains. Personal information may be collected from referral sources with the prior consent of the individual.
5. Limiting, Use, Disclosure and Retention Recovery Centre for Addictions, Trauma and Families will not use or disclose personal information for purposes other than those for which it was collected except with the consent of the individual. Personal information shall be retained for the duration of the service and for five years thereafter or as required by law or contract with a funder or client.
6. Accuracy Personal information shall be as accurate, complete and up to date as necessary to fulfill the purposes for which it is to be used.
7. Safeguards Personal information is kept in strict confidence. All client and service information is governed by the Client Confidentiality policy. This policy authorizes employees and volunteers access to personal information on a need to know basis. It also provides for the secure storage and handling of personal information.
8.Individual Access Recovery Centre for Addictions, Trauma and Families shall, upon request, inform an individual of the existence, use and disclosure of his or her personal information and shall be given access to that information according to the Client Confidentiality policy.
9.Openness Recovery Centre for Addictions, Trauma and Families shall make available to individuals information about its procedures, practices and policies relating to the management of personal information.
*The above policy was adopted and designed to comply with the Code of Ethics of the Professional Association of Canadian Christian Counsellors (PACCC) Excerpts from the Code of Ethics of the Professional Association of Canadian Christian Counsellors Privacy I.32 Explore and collect only that information which is germane to the purpose(s) for which consent has been obtained. I.33 Take care not to infringe, in research or service activities, on the personally or culturally defined private space of individuals or groups unless clear permission is granted to do so. I.34 Record only that private information necessary for the provision of continuous, coordinated service, or for the goals of the particular research study being conducted, or which is required by law (See Standards IV.15, and IV.16). I.35 Respect the right of employees, supervisees, students, or Christian counsellors interning to reasonable personal privacy. I.36 Store, handle, and transfer all records, both written and unwritten (e.g., computer files, facsimiles, video-tapes), in a way that attends to the needs for privacy and security. This would include having adequate plans for records in circumstances of one's own serious illness or death. I.37 Take all reasonable steps to ensure that records over which they have control remain personally identifiable only as long as is necessary in the interests of those to whom they refer and/or to the research project for which they were collected, or as required by law, and render anonymous or destroy any records under their control that no longer need to be personally identifiable. Confidentiality I.38 Be careful not to relay information which they have gained about colleagues, colleagues' clients, students, and members of organization gained in the process of their activities as Christian counsellors and which the Christian counsellor has reason to believe is considered confidential by those persons, except as required or justified by law (see Standards IV.15 and IV.16) I.39 Clarify what measures will be taken to protect confidentiality, that there are limits to confidentiality and what responsibilities family, group, and community members have for the protection of each other's confidentiality, when engaged in services to or research with individuals, families, groups, or communities. I.40 Share confidential information with others only with the informed consent of those involved, or in a manner that the individuals involved cannot be identified, except as required or justified by law, or in circumstances of actual or possible serious physical harm or death (see Standard II.36). Self-Assessment Responses All responses you make to the questions in the self-assessment test are completely anonymous and confidential. For the purposes of scientific research and usage analysis, your responses will be associated with an anonymous numerical identifier that will not be linked to browser cookies or your IP address (see below for information on cookies and IP addresses). If we publicly release user response data, it will only be in aggregate (group) form.
Email Addresses Email addresses you submit for the "Tell a Friend" feature will be used only to send informational email about addictionscounselling.ca to the addresses you specify. Your name and email address will only be used to notify you of any mailing errors and to identify you to the recipients you specify. Under no circumstances will your email address or that of your recipients be added to any mailing lists. Email addresses will not be sold, traded, given or otherwise distributed outside Addictionscounselling.ca.
